Root->128 accuracy audit re-review pass (#5903): fixed 2 missing probe blocks; diffed the 99-path OpenAPI schema and added 2 new surfaces (readyz, Prometheus metrics) after live-testing every no-param candidate -- most declare no security but are actually 401-gated in practice.
